Appeal a Discharge or Denial of Coverage

If a health care provider decides that the services you are receiving will no longer be covered by Medicare or you were denied admission to a hospital, and you disagree with that decision, you can request an immediate, independent review of that decision. Stratis Health does those reviews for Medicare consumers in Minnesota.


We’ll make a determination if we can help or will provide a referral to who can help.

Hospital discharge appeal line

If you think you are being asked to leave the hospital before you are medically ready, first talk with your doctor about your concerns. Your hospital will provide you with a written notice of your Medicare rights. This notice is called “An Important Message from Medicare” and informs you of your right to appeal (formally question) the decision to be discharged.


Call Stratis Health's Hospital Discharge Appeal Line toll-free at 1-866-894-1327 if you decide to appeal this decision. This line is answered during business hours, 7 days a week. If you call after 4:30pm and leave a message, we will return your call the following morning.


Non-hospital immediate appeal line

If you are receiving care from a home health agency, a nursing home, a comprehensive outpatient rehabilitation facility, or a hospice, and your caregiver informs you that Medicare may stop paying for your care—issues a notice of non-coverage—call Stratis Health’s Non-Hospital Immediate Appeal Line at 1-877-624-1414. This line is answered during business hours, 7 days a week. If you call after 4:30pm and leave a message, we will return your call the following morning.
