Our staff is adept at tailoring educational offerings to suit the needs of our clients and health care providers. We work to determine the most effective means to further knowledge and enable action.

Training sessions and conferences

We develop and offer training sessions and conferences, on our own and with many partners across the state. Our Web site has numerous Webinars posted for individual education purposes or for sharing as part of an educational session at your organization. Collaboratives

Stratis Health has found that learning collaboratives provide an excellent environment for organizations to explore the myriad of possible approaches. A collaborative is a change process model that brings together a number of organizations focusing their improvement work on similar goals. It builds from the Institute for Healthcare Improvement’s Breakthrough Series designed to accelerate improvement in health care, using a structure for learning and action—with an emphasis on action.

• Minnesota Rural Palliative Care Initiative. In fall 2008, Stratis Health started facilitating a learning collaborative, through which 10 community teams will start or strengthen palliative care programs in their communities.
• Home Health Collaborative. The 62 home health agencies in a Stratis Health learning collaborative funded through the Centers for Medicare & Medicaid Services from 2005 to 2008 reduced avoidable rehospitalizations by 6.3 percent—resulting in improved care for seniors and a significant savings to Medicare.
  

Toolkits

We have extensive experience in developing toolkits to assist health care providers in improving the care they deliver. Over the past 10 years, Stratis Health has developed more than 25 kits disseminated locally and nationally—covering various clinical and operational topics, as well as targeting different settings of care. The toolkits are actionable and evidence-based. Examples include:

• Health Information Technology Toolkits for various settings of care (clinics, Critical Access Hospitals, and nursing homes).
