Health Information Technology Toolkit for Home Health AgenciesThe Health Information Technology Toolkit for Home Health Agencies helps home health agencies, private duty nursing services, and home-based hospices, and other organizations providing health care services in the home assess their readiness, plan, select, implement, make effective use of, and exchange important information about the clients you serve. The toolkit contains numerous resources, including tools for hospice, telehealth, health information exchange, and personal health records. If you offer services such as hospice in another location, like a nursing home or hospital, refer to Stratis Health's toolkits for these types of care delivery organizations. A wide variety of HIT is available for use in home health. The HIT you choose to adopt may range from independent applications that start you off on using HIT, or are those that supplement what you already have; or they may be a comprehensive system for integrating all business and clinical aspects of home health. A carefully constructed foundation and the right tools can mean the difference between systems that are not well-used or even add administrative burden, and those that achieve value. The purpose of this toolkit for is to provide tools, tested in this environment, that will help you plan and make the right choices, as well as to avoid having to re-invent the wheel as you plan for and implement HIT in your home health agency. Much like a carpenter's toolbox, the HIT Toolkit supplies a variety of tools for use at different points in your HIT adoption. Each tool includes:
