Medicare Quality Innovation Network - Quality Improvement Program (QIN-QIO). (2014-2019) Stratis Health leads Lake Superior QIN, as part of a national network working to achieve the Centers for Medicare & Medicaid Service's (CMS) quality goals. Lake Superior QIN works with communities, health care organizations, and individual providers in Michigan, Minnesota, and Wisconsin on better patient care, better population health, and lower health care costs through improvements. CMS Quality Innovation Network National Coordinating Center. (2014-2019) As a partner with Telligen, Stratis Health supports QIN-QIOs across the country to advance CMS' new approach to improving health care for Medicare consumers, families, and caregivers nationwide. Our work focuses on nursing homes and enhancing electronic health record (EHR) use.

Rural Quality Improvement Technical Assistance (RQITA). (2015-2018) Through this program of the Federal Office of Rural Health Policy (FORHP), Stratis Health provides technical assistance related to quality reporting and improvement to beneficiaries of FORHP quality initiatives across the country such as grantees, CAHs, and other rural providers. Adverse Event Assessment and Training. (ongoing) Assessing the root cause analysis and corrective action plans submitted by hospitals and other providers as required by the Minnesota Adverse Health Events Reporting law, and identifying and meeting training and measurement needs. More >

Cultural Competence. (ongoing) The Culture Care Connection online learning and resource center helps clinicians better communicate with and engage patients from diverse populations to improve their health outcomes and care delivery. Supported by UCare.

Advance Palliative Care in Rural Communities. (2017-2019) Stratis Health has launched a multi-state project to increase access to palliative care services in rural communities. The project aims to build local capacity to cultivate palliative care programs, frame services within emerging payment models, and explore how technology can enhance the quality and efficiency of services. Regional Extension Assistance Center for Health Information Technology (REACH). (2011-2016) Stratis Health co-led REACH—a nonprofit federally designated HIT Regional Extension Center. We helped providers in Minnesota and North Dakota improve care by implementing and using EHR systems. REACH served nearly 5,100 clinicians at 662 clinic locations, and assisted 121 critical access and rural hospitals to achieve Stage 1 meaningful use. More >

Medicare Quality Improvement Program (QIO). (2011-2014) Stratis Health was part of a national network of 53 QIOs working to achieve CMS quality goals. Stratis Health worked with communities, health care organizations, and individual providers in Minnesota on better patient care, better population health, and lower health care costs through improvements. CMS Special Innovation Project: Health Information Technology for Post Acute Care Providers. (2012-2014) We assisted post acute care providers in Minnesota—focusing on skilled nursing facilities—to improve quality and coordination of care through the effective use of HIT during care transitions, leverage standardized patient assessment content to facilitate health information exchange with hospitals, and reduce medical errors by improving the medication management process. National Nursing Home Quality Assurance Performance Improvement (QAPI) Program. (2011-2013) In partnership with the University of Minnesota, Stratis Health lead development of a national QAPI program for nursing homes, on behalf of CMS. The program established standards and provided tools and resources for nursing homes to develop best-practices to meet the standards. More >

State Innovation Model (SIM) Minnesota e-Health Roadmaps. (2015-2016) In conjunction with the Minnesota Department of Health (MDH), Stratis Health developed roadmaps that describe a path forward for providers to effectively use e-health to participate in the Minnesota Accountable Health Model for the settings of long-term and post-acute care, local public health, behavioral health, and social services. Cultural Competence. (2012) More than 70 clinics and four public health agencies across Minnesota received our consultation and technical assistance to measure and enhance the cultural competence of their staff working with multicultural patient/client populations.

RARE Campaign. (2011-2012) Stratis Health as Minnesota's QIO, the Minnesota Hospital Association, and the Institute for Clinical Systems Improvement led RARE (Reducing Avoidable Readmissions Effectively), a statewide campaign to prevent 4,000 avoidable hospital readmissions across Minnesota by the end of 2012. Rural Palliative Care Community Development Project. (2012-2014) Stratis Health worked with seven rural Minnesota communities to improve palliative care, involving more than 40 organizations in an effort to establish or strengthen palliative care in rural areas of the state. To date, 25 communities have been part of our rural palliative care initiatives. More >
