Minnesota e-Health Roadmaps to Support Timely and Effective Care for Minnesotans

The Minnesota Department of Health (MDH) and the Minnesota e-Health Initiative have partnered with Stratis Health to develop the Minnesota e-Health Roadmaps. The roadmaps will describe a path forward and a framework for providers to effectively use e-health to participate in the Minnesota Accountable Health Model for the settings of long-term and post-acute care, local public health, behavioral health, and social services. The project will begin in January 2015 and end in the spring of 2016.

This work is made possible through Minnesota’s $45 million State Innovation Model (SIM) testing grant. MDH and the Department of Human Services (DHS) jointly received this federal grant in 2013 and are using the funds to help implement the Minnesota Accountable Health Model. The purpose of the Minnesota Accountable Health Model is to provide Minnesotans with better value in health care through integrated, accountable care using innovative payment and care delivery models that are responsive to local health needs. The approximately $600,000 contract was awarded to Stratis Health after a competitive Request for Proposal process.
