Health Plan Performance Improvement Project: Pneumococcal Vaccination

Three Minnesota health plan—Medica, Metropolitan Health Plan, and UCare—collaborated to help low-income, community-dwelling seniors receive pneumococcal vaccinations. The health plans worked with Minnesota Immunization Information Coalition to ensure the availability of accurate member vaccination data. The group also focused on educating members about the importance of pneumococcal vaccination, as well as, ensuring that care coordinators helped to arrange clinic or home-based vaccinations. This project ended in 2009. The final data analysis indicates that with each measurement, the number of eligible members with a health plan claim for pneumococcal vaccine increased. The final measurement, 40.9 percent, shows an improvement of 15.9 percent over the baseline rate of 24.9 percent.
