Health Plan Performance Improvement Project: New Member Utilization of Preventive Care

Four health plan—HealthPartners, MHP, Medica, and UCare—have joined forces to increase the number routine physical exams received annually by adult members enrolled in Minnesota Health Care Programs. Members are encouraged to schedule a preventive visit (annual physical exam) within the first six months of enrollment into the plan. This project was designed to promote the benefits of preventive care to new members through direct education and collaboration with providers. Members are encouraged to establish and maintain a relationship with their care team, including their primary care providers and primary care clinics. The project is being evaluated using health care claims.
